Your Care Journey

Understanding Your Home Health Episode

Home health care is provided in structured 60-day episodes under your physician's Plan of Care. Here's what that means for you, and what to do if you're ever hospitalized during your care.

Each Care Episode Is 60 Days

Home health services are organized into certification episodes of two months (60 days) each. During this time, your care team follows the Plan of Care ordered by your physician.

Day 1

Episode begins & SOC assessment

Day 30

Ongoing visits & monitoring

Day 60

Recertification decision point

Recertification for Continued Care

At the end of each 60-day episode, your Registered Nurse (RN) will complete a recertification visit and reassessment. If your physician continues to see a medical necessity for skilled home health services, your care will be recertified for another 60-day episode.

  • The RN reassesses your condition, progress, and ongoing needs.
  • Findings are reported to your physician for review.
  • If medical necessity continues, a new Plan of Care is ordered and services continue without interruption.
  • If goals have been met, your physician may determine you are ready for discharge from home health services.

Important

If You Are Hospitalized During Care

If you are admitted to the hospital for any reason while you are an active Instant Care Home Health patient, please let the hospital staff know the name of your current home health agency as soon as possible.

1

Tell your hospital care team or case manager that Instant Care Home Health, Inc. is your current home health provider.

2

This allows the hospital's discharge planning team to notify us directly about your admission and expected discharge.

3

Once notified, we can coordinate a smooth Resumption of Care (ROC) visit after you return home, so your services continue without delay.
